We can see this lesion in cases of cachexia. Due to several reasons, there can be a mobilization of the fat stores, such as that on the epicardium. The lipid vacuoles reduce their size and are replaced by a proteinaceous fluid, with a simultaneous increase in the interstitial fluid, developing a greyish gelatinous appearance. We can sometimes see small white fat necrosis spots.
We must make a differential diagnosis with oedema, whether with an infectious aetiology or not.
